401(k) On-line enrollment and Election changes

For those of you who have not enrolled in our 401(k) plan, if you would like to enroll in the future, you may do so on-line. Please remember that all employees are eligible to make contributions, however, you are not eligible for the 3% company match until you have completed one year of employment.

If you have already enrolled in the 401(k) plan, but would like to make changes to either your contribution percentage or investment selections, you may also do that on-line.

To take advantage of on-line 401(k) management, go to Paychex. Please follow these instructions, as they apply to you:

Currently registered for on-line FSA Access through Paychex
· Log-in using your user name and password already set up
· Select Retirement Services
· Choose “Enroll Now”
· Verify Date of Birth
· Select Investments
· On the confirmation page, verify info and select “Submit”
· Receive confirmation receipt – before leaving this page, print the beneficiary form to return to Andrea for your personnel file

Not already registered for Paychex on-line access
· Choose “Register for a new account”
· Create account by filling in the fields, then choose “submit”
· Will return to homepage, where you can select Retirement Services
· Choose “Enroll Now”
· Verify Date of Birth
· Select Investments
· On the confirmation page, verify info and select “Submit”
· Receive confirmation receipt – before leaving this page, print the beneficiary form to return to Andrea for your personnel file
